Data-driven decision-making in Hillingdon’s PCNs, backed by digital solutions, improved access to care by using real-time patient data to identify unmet needs and gaps in clinical care. Key data such as defining specific need of activity as well as the team member who could undertake the work helped inform resource allocation and service planning. Linking this data to the Quality and Outcomes Framework (QOF) as well as Local Enhanced Services allowed for targeted interventions, like improving diabetes and mental health management. Regular monitoring of all contract-related indicators ensured that care remained patient-focused, driving performance and financial incentives tied to better health outcomes.
