The main driver of the social prescribing link worker model is recognizing that we need to move from admiring the problem to doing something about it. Evidence shows that the health and well-being of individuals and communities are primarily dictated by a range of environmental and socio-economic factors. These factors are driven mainly by social injustice and inequalities. These unfair disadvantages are reversible and impact the overall healthcare system and quality of life. Social Prescribing Link Workers play a huge role in this regard.
