Dan is a Consultant Community Geriatrician and Strategic Clinical Lead for Frailty in Hull and the East Riding of Yorkshire. Dan and colleagues have worked collaboratively with the ICB, health and social care providers to deliver an award-winning community frailty model based at the Jean Bishop Integrated Care Centre, Hull. Since 2018, the specialist multi-disciplinary team have delivered proactive comprehensive geriatric assessment for people living with frailty resulting in a sustained improvement in patient and system outcomes. They have also implemented a 7-day Urgent Community Response (UCR) model and Frailty Virtual Ward which provide safe and effective community-based frailty care. Dan is also Clinical Lead for the Humber and North Yorkshire ICB Community Centre of Excellence for Frailty, a new initiative to drive a standardised approach to the prevention, identification and management of frailty across the ICB.
