I have been working in Primary care for nearly twenty years and Manage the Nurses and Allied Health professionals Team. I have been working with people with diabetes one to one for most of this time. six years ago we introduced Group Clinics for people with Type 2 Diabetes. Since then we have expanded to include groups for hypertension and COPD. Group clinics are a way to join people with similar health issues and consult with the clinician and peers to discuss and share ideas to improve self management.
I know work with ELC to teach others how to use the group model to work with people to improve health.
